the standard method of calculating the attrition rate is no:of employees who left during the year /average no. employed for that year
and HRM dept should discuss with the employees who left the org as to wat were the reasons of their leaving the org..and maximum attrition rate is after the performance appraisals of the concerned employees..
attrition can be due to various problems to quote a few it may be due to conflict wid boss,,no career growth,compensation issues..

but i would like to add some more points to her say....
Attrition rate is actually calulated on a yearly basis......but there are many companies which also calculate their attrition rate on monthly or quaterly basis.....
Here i am sending some documents where in you can find how the attrition rate can be calculated and also the format (in the excel format)......
Based on these documents you can then make your report on what is the attrition rate of your company and what steps can be taken or you would suggest to reduce the attrition rate....
