A NMI happened in my plant in august but I was out of touch, so could not share earlier. i actually never expected that ever I'll get such NMI reported. It is not a very big incident but we must take care of these kind of incidents too. This took place in DM plant of CPP.

Dear Inder,
I would call this incidence as Minor Accident.
I have seen many accidents of the similar nature.
In one of the case the treated effluent was collected in a drinking water bottle (plastic bottles)
This bottle was kept near the table of concerned manager which was to be sent to Laboratory for analysis.
One visitor came to this officer and tried to drink this water.
In both the cases full proofing is necessary.
Following actions can be taken in your case.
1. The DM water and Service water shoul not get mixed.
2. There has to be indicators in local language saying "NOT FOR DRINKING"
3. Awareness to all employees
4. Restricted entry to visitors or those who are not concerned
5. The drinking water tap should not be kept in nearby areas. (even water coolers)

As said by Mr. Sudir, it is an incident.
If the valve passing is found earlier & then if you had initiated online monitoring only it can be classified as a near miss. However, when some thing has happened, it is an incident.
Kindly change this near miss to an incident.
