Here is a very basic outline for a questionnaire for behavioral trainers:
NEEDS ANALYSIS INTERVIEW
YEAR: 2007
NAME:
DEPT:
DATE:
1. What should be the objectives for the training program (AOI for the employee)?
2. How does the identified area pose a problem in performance currently?
3. What symptoms are observed? (Examples where skills/behavior/knowledge are lacking)
4. What are the probable causes? (Attitudes/Behaviors/Systems/Policy/Non-clarity/Unconscious, etc.)
5. What will be the training implications? (Expectations after a training program)
6. What are the non-training implications? (Aspects that a training intervention cannot affect)
7. What situation/problem/issue needs the most attention?
