PF withdrawal process shall be done by your employer. If you want to try yourself, you should fill out the following forms:
1) Form 10C
2) Form 19
3) Electronic Transfer Form
These forms should be authorized by your employer. Along with those forms, you should enclose a cancelled cheque leaf as proof of your account number.

Last payslip and relieving letters are not required for the PF office. Anyhow, the employer gives a declaration for the date of relieving in both Form 10C and 19. That is the proof for relieving. Instead of withdrawing, it's better to get it transferred to the existing account.

In some of the states, like in Himachal Pradesh, the regional PF commissioner insists on sending a cancelled cheque of your account mentioned in the claim form or a photocopy of the passbook of the account. Otherwise, he rejects the claim by putting forth the above query.

Dear All,
Please clear the matter. I worked with my previous employer for around 9.5 years. In 2007, I resigned from the company and withdrew my PF. My EPF part balance is pending, and the employer informed me that I would receive the scheme certificate. Despite several follow-ups, I have not yet received the scheme certificate. Recently, when I contacted my previous employer, they mentioned that I should have received Form 13 to transfer the EPF parts from my previous employer to my current employer.
Could you please confirm if this is possible?
