You have to collect all the required details for Form 1 and enter the same in an Excel file before opening the ESIC webpage. Search for the option to import from the Excel file and import all details in it, then upload it.

From the ESIC Portal, you can download a sample Excel template. There you'll get the instructions and an empty Excel sheet. You just need to fill the Excel sheet as instructed and upload the data from the Excel file when you're online. For any queries, you can follow the instructions provided. I believe it will help you complete the work very easily. You can find the file in the "FILE MONTHLY CONTRIBUTION" option.
