Am Shabana, Sr. HR Executive in an IT company @ Pune.
I am working on preparation of Appraisal forms for Non -IT staff (Admin, Acconts etc..)
Want to prepare a form wherein the comments of both the Appraiser and Apprisee can be mentioned.

first prepare a detailed job description, analyse whether the candiate is effective/ effiecient.
next for subjective criterion use a ranking method ie 1-5, ask them to give detailed instances if scores are on the xtreme.
ask the candiate to fill it up, nexty discuss it wth the boss and finally make both of them sit together and thrash it out if they disagree
