I believe there is no specific format to follow when applying for leave; you can write in your own style. For example, you could write something like the following:
Dear Sir,
I would like to request leave from [start date] to [end date] for personal reasons or any other purpose you may deem fit. Therefore, I kindly ask for your approval of my leave.

Leave letters are the first 'application letter' format taught in schools. I think ignoring one's school education adversely impacts the whole personality of an individual. I hope parents will keep this in mind to ensure all-round development of their children.
Wherever in any organization, there are no forms/formats/leave-books for applying for leave, one's school education would come in handy. One need not be a graduate just to write a letter/application for leave.

It's a very simple funda that if you are looking for a 2-day leave, just call your boss and say, "Boss, I have viral fever and am unable to attend the office for 2 days." It is understood that viral fever usually lasts for 3 to 5 days. I think this is the best format for your leave.
