I have worked in an Ltd. organization for 3 and 1/2 years and left that organization on 16th Nov. 2009. Now, I want to withdraw my PF and EPF amounts. I want to know:
1. Am I eligible only for the PF amount, and will my EPF amount go to waste?
2. I don't have the latest slip, so how can I find out my actual money?
3. What is the total amount of my PF and EPF since we have no receipt of the EPF amount?
4. Are the receipts of PF required to be shown while withdrawing?
5. Do I have to fill out two separate forms, or is only one form for withdrawal sufficient? Which form is applicable?

Forms Required for PF Withdrawal
You have to fill up the following forms and submit them to the PF office:
• Form-19
• Form-10(c)
• Form-10
• Form-5
• Form 3-A (Revised)
Finding Out Total Earned Money
To find out the total earned money, you have to go through the salary sheets of your previous organization. They should help you.

I have the same problem as Mr. Manish. I filled out Form-19 and Form-10C. Are the other forms you mentioned necessary to provide, or are they just supporting documents? (Form 10, Form 5, and Form 3A). Please advise.

You are eligible for both PF and EPS amounts. If you have a service of 3 years and 6 months or above, it will be rounded up to 4 years. If it is below 3 years and 6 months, it will be rounded down to 3 years. The factor stipulated in Table D for 3 years is 2.98 and for 4 years is 3.99. This will be multiplied by the average salary of the last twelve months to get the withdrawal benefit towards EPS/Pension Fund.
