I need a certificate of identity ( or identity certificate) letter format -- with photo -- for PAN card address proof correction. The letter needs to be signed by a Tehsildar. I checked out in the internet for this template format but none of them (including TIN NSDL..) have the sample form for this. The concerned person in the taluk office informed me to get the letter format with details without which he cannot sign in and issue the letter. I am totally confused about this and have no idea how the format should be. Could any one kindly help me on this. The letter should contain details as follows:
1) Name -- self
2) Father's Name
3) Date of Birth
4) Residential address
Could anyone kindly reply ASAP as i need to reply to the PAN office at the earliest?

Hi,
I have attached doc format used to obtain the pan card ( for address correction..). Hope you can use it with minor changes as per the requirements for bank account. Please note that this has to be attested(get signature) by a registered Gazzeted Officer.
