Excellent MIS format, you can also incorporate Budget V/s Actual Personnel cost, Training Man hours Department-wise, Statutory Compliances column if required.

Good work done. All these details look like extract from Employee Master. Here are some improvement points:
Statutory compliance: The list of various statutes is shown in the format. You can also add the details of compliance, such as Registers and Returns to be maintained/sent to various authorities, periodicity, due date etc to make it more comprehensive.
Contractors Details: Two important aspects in Contract Labour (Regulation & Abolition) Act are getting the Registration Certificate by the Principal Employer and obtaining license by the contractor. Hence, we can also include the License number and the validity period. We can also categorize licensed contractor (those who engage more than 20 and unlicensed contractors. In case of ESI notified areas, we can have ESI code instead of WC details.
In the Accident details, we can also capture the number of mandays lost due to each accident.
We can also add Absenteeism, Employee Cost of Regular Manpower, Flexi staff also in the MIS Report.
Various Employee Related cost, such as Recruitment Cost, Training Cost, Transport, Canteen, Telephone etc, which are managed by HR/Personnel may also be part of HR MIS Reports.
