Dear Sir
The reasons behind not usage of PPE's are as follows:
1. Overconfidence
2. Provide inferior quality PPE's by employer to their workmen
3. Inadequate training
4. Lack of enforcement form management
5. Lack of supervision
6. Work pressure
7. Availabilty of resources.
This much only coming to mind now...

let me too add up some.
1. as rightly said by dipil, Overconfidence.
2. not feeling comfortable.
3. supervisors not providing them
4.feel it burden & useless,coz they argue that does helmet gaureentees that my head is safe , like this
5. stupid excuses, like i just now removed it.
6. lack of knowledge about correct PPEs & procedures to use.
7. Behaviour.

I would like to something more on my commend:
The lack of supervision is itself the prime reaon... The supervisor only allow a person to strat the work without required quality PPE's... Further if the supervisor put enforcement in place, then the non-use of PPE may not even come...
Secondly if the worker is new to the industry or from a village, he may not be use to with the PPE's. This also can be one of the contributory reason...

Dear Dipil.
You have some good points and your appraoch is in the right track but it is incomplete.
What we need here is a safetyman's perfect or near perfect answer. Often others look at safety people only for advice. He is supposed to have the last word in safety and he should be equipped with it.
.We can wait for little more time to close it.

Adding one more point ot my answer please:
Lack of understanding the responsibility towards safety, may be one of the another reason. Because most of the workers, thinks that looking after safety and making the job safe; is the job of safety officers, supervisors & engineers. In this case they (workers) fail to understand the concept that safety is individuals responsibility. By providing quality PPE's to the employees the emploer is in safe side, now its their responsibility to make the right use of it. Simply can say the concept of "Accountability" to be percolated down the level, upto the worker. I think the concept of accountability, we have discuss in this same forum as a reply to one another posting of you.

Basically it is attitude. People feel that it is for the company. Major cause is that the superiors don't wants to be role models, they will ignore and they want the juniors to comply.

In my career I have seen one great leader, in a Pharma company every individual who are entering in to the work sport need to wear the uniform along with the cap. The chief is very aggressive, one day he entered the work spot with out a cap, most of the people were observed but unable to tell him, one employee straight away went and told him that he is with out cap in the work spot. Every body felt that this guy will be fired but to the surprise of all he went out and returned back with a cap on his head. At 5pm in the evening there was a gathering and in the meeting the Boss tendered appologies to all for entering the work spot with out a cap and given one small gift ( procured with his own money ) to the employee who has reminded him. I have not seen him with out cap any more in the work spot not only he the supervisors also. Finally it is the commitment of the superiors for implementing and the punishment system also should be very stringent in Indian scenario.
