What is the method using performance appraisal of Manager/Supervisor by his subordinates? Is there any separate form available? Kindly send it if anybody have the same.

Dear Sellakumar SP,
In your case what matters more is maturity of the subordinates. Are they capable to appraise their superiors? Has any training been conducted on how to conduct the performance appraisal of superiors? Is it that you wanted to bring 360 degree appraisal in your company?
There are always perception gaps between superior and subordinates. What efforts have you taken to bridge those gaps?
I request you to concentrate on these larger issues. Apprailsa Form is something smaller.
Appraisal form can be same. It need not be different.
