Dear Sir,
Regarding human beauvoir ,it is only required for hazards/risks . Not all activities will involve human behavior , that is unsafe act . Take the example of work at height, the permit to work system and safety belt will not guaranty the risk level because the people will not use the safety belt by fixing to any structure which is a common phenomena. This kind of requires human element which should be considered the risk level. So identify such activities and give due risk level .

Kindly check for new amendments.
Also attaching for your reference.
Even Factory act and Petroleum Act speaks of it but it should be as per "Static-and-Mobile-Pressure-Vessels-Unfired-Rules-1981"

the study on aspect / impact related to environment and hazard / risk related occupational health for the activities are derived pre defined matrix. what type of matrix you are using? is it common for both types or you have different for different areas.
standarad type recommended by the consultants may be SSPD (scale, severity, periodicity and duration) for environmental matrix and likelyhood of occurance and hazard severity for occupational health.
which one are you using? try to customize the scale for likely hood, severity like factors to address your concern on human behavior. aslo if can co relate this abnormal human behaviour to training, education, skills etc you may address the issue better.
