Hello KVE:
Will the feedback be anonymous?
Are the team members prepared to be honest and will they be honest?
Will the manager know who says what about him?
Will managers retaliate against team members for poor ratings?
Don't take chances with managers and their direct reports, we can do more harm than good.
For our clients that want such feedback we provide a web based 360 that has 70 questions and the direct reports' ratings are anonymous so that managers do not know who says what about them. We used to use paper and pencil rating forms but all too often the forms were stored in house and eventually some managers got to see who said what about them which is not a good idea.

How the feedback form will look and the process you use to disseminate it, analyze and feedback results will very much depend on what you want to achieve with the feedback – and on the culture of your workplace. As Bob Gately says, feeding back to managers can blow up in your face if not handled professionally.
There are a number of good online 360 degree employee feedback options available. Things to be especially mindful of are:
-question design
-participant confidentiality
-manager debrief
If not handled well, managers may end up feeling angry, disrespected and disenfranchised. This will then have a flow on effect to their direct reports. I can only say: proceed with caution and get professional advice.
