The plant security report is also called the "Daily Occurrence Report". You can design the form on the following points:

a) The number of visitors for the day
b) The number of material returnable gate passes issued by the user departments for the day
c) The number of material non-returnable gate passes issued by the user departments for the day
d) Observations about perimeter security
e) Observations about material security
f) Observations about the electrical assets installed in the plant
g) Observations about the physical search of the employees conducted during the day
h) Details of the fire incident if any
i) Whether any new theft case is reported by any department during the day.
j) Whether the investigation is in the progress of the past theft case(s) and their expected date of completion
k) The number of security personnel sanctioned per shift Vs actual worked
l) The number of security personnel did double shift during the day
m) Is there any need to keep any employee or the department under security surveillance? If yes, then mention the reasons.
n) Is there any untoward incident related to the vehicle or the transport department?
o) Did security personnel overtly or covertly observe fights among or between the employees? If yes, then provide the details.
