Ours is a construction company. Our principal employer received a notice from Labour Department, Haryana to submit "Record of worker beneficiaries/registration in Form-V of all workers on site working with each contract". Can any one provide the format or details to be included in the form? Can any one tell whether it is a form or register?
Please help in this matter.

This is for your information that this notice has been given by the Labour Department ( Factory wing) for compliance under The Building & Other Construction Workers Act, 1966 & The Haryana BOCW Rules, 2005 for submission of Form V ( Registration form) & the record of beneficiaries. Record of beneficiaries is a format having details of particulars of the regd. beneficiaries & Form V is an application for registration for becoming entitled to have benefits of the schemes declared by the The Haryana BOCW Board.
Ar required both the formats are attached herewith for your ready reference.
