I am currently having one on one discussion with all my employees and I would like to document the discussion held so far in a format or excel, after that I could be able to generate a presentation on the status, eg. Satisfaction level, training needs, feedback about workplace, feedback about management, feedback for HR improvisation. Request if you have any template for me to begin with, so I would be able to document maximum for future benefit of my whole organization.

Thanks for sharing questionaire, it is really an explicit data.
As of now I will be having a one on one discussion and its actually not related to question answer round, discussion on job related task.
I need to prepare a report after these discussions and actually need a ppt format where I can present to management statistics of my HR discussion with employees and suggestions for betterment.
