This is a humble suggestion in connection to your Query.
I have gone through your Query and seen the Word document. Here are a few suggestions which may increase responses from fellow HR practitioners .. I have included a Sample MS Excel sheet which you may open for your reference ..
1. Do not enlist all 25 Competencies in one go .. less than 0.1 % practitioners will be able to remember the actual competencies in use. (You may use the format of Excel sheet as in the 'competencies' tab which I tried to translate as per your Word format) ..
2. In your word document Page 3, if a person wishes to enter response for your Q. No. 7, the formatting will get distorted. Probably that can be made simpler using Excel, sheet 1 - Basic Info - Contact Details field, for example.
3. Please consider revising the format and individual questions .. the problem with a typical HR practitioner is too much paper work. If you can make it simpler and interesting people will respond .. e.g. Instead of asking "Can you list some of training measure you follow, if you find some gap about various predetermined competencies?
(Please write them as shown in the box.)" - write it (example) "Training measures used after Competency Gap Analysis .." A common man would love simplicity, isn't it ?
Just shared my own experiences with you ..

Would you mind if I wish to know the upcoming (broad) Areas of research available in DSE as of now ? (I have completed MA in Econometrics and then Masters in HR from a Central University of India, I shall be thankful if you can help me with that ..)
