I am in the process of terminating an agreement with my vendor following a breach of the said agreement.
Is there any template or format I can use for this purpose? I need to issue a one month notice as per agreement.

Firstly, you should go through the terms laid down in your agreement as to whether the vendor has made any breach of any terms or not. Otherwise you shall have to stick with the terms & conditions laid down in your agreement and accordingly you can serve one month notice of termination.

Review of terms and agreement.
Especially check what the termination clause says.
Check the breach of agreement is justified as per the agreement terms.
Check for notice period towards termination of agreement.
Check if the vendore has cleared all compliance till previous period.
Also wouldd suggest giving an oportunity to vendor to be heard and explaination if any.
If all the above is done you can draft nice termination letter with appropriate notice period and indemnity for smooth withdrawal of services etc.

There is no specific format or template required by law.
Google for copies of letters or format that others have used.
You need to give a simple notice of termination in the form of a letter stating the termination, referring to the relevant clauses of your agreement and giving the reason. Ensure date of termination is specified, what happens to existing work and employees and pending invoices / settlement of dues. You can say “settlement will be done after compliance’s on your part are completed” or something of that nature
