Dear Friend,
I think there is no specific format to follow for applying leave. You can write with your own.
for example you can write like below.
Dear sir,
I would like to take leave form ?????? date to ????????? due to attend personal work or what purpose you want to take . Hence I request you to please approve my leave.

yours faithfully.
you can right better than above, this is just for an idea.

There is no hard and fast rule as such. Every company has format of its own wherein an employee fills the same and get it approved. Even if there is no format per se, you write it on your own - have not you been trained in your school (as to how to write leave letter).
Please post something which is very ticklish and you are not able to solve with your experience, so that others can come for rescue.

Leave letters are the first 'application letter' format taught in schools.
I think ignoring one's school education adversely impacts the whole personality of an individual.
I hope parents will keep this in mind, to ensure all-round development of their children.
Wherever in any organization, there are no form/formats/leace-books for applying for leave, one's school education would come handy. One need not be a graduate, JUST TO WRITE A LETTER/APPLICATION FOR LEAVE.

Its very simple fundda that, if you are looking for a 2 days leave, just call your boss and say "boss i have viral fever and unable to attend office for 2 days" it is understood that viral fever goes for 3 to 5 days. I think this is best format for your leave.
