I need to withdraw money in my Provident Fund. It has been 11 months since I left my service. I resigned because of marriage reason. Now I need to fill
form 10-C and have some doubts:
1. What is PPO?
2. What does the wages mean? Does it mean Basic salary?
3. What does period of non-contributory service mean?
4. Can i send the scanned copy of the filled form to my relatives and they can take the print out and send to the company? Is the scanned copy acceptable?

In your case since service rendered is only 11 months PPO # is not applicable, this is applicable to those employed in service before 1995 EPS scheme.
Wages means in PF terminology your Basic + DA (if applicable) on which pf deduction is made.
Period of non-contributory service means if theres any break in service continuity
last point only originals will be accepted by any PF office, in your case scanned copy will not suffice.
