Dear Ravi Indurkar,

Thanks for sharing the "Interview Assessment Form". However, just a small clarification. Why to ask the details of the family members at the interview stage? What purpose does it serve?

Secondly, who fills this form? Will it be filled by the candidate or by the recruiter? If the latter, then will it not impose additional data entry work of the recruiter?

Hi,

The data ( till Column C) needs to be filled by the candidate in the personal data form before writing the technical test.

The Interview Assessment Top Sheet should contain assessment of attributes like

Job Knowledge
Ability to express (Communication)
Self Confidence
Positive attitude / Enthusiasm
Analytical skills
Knowledge of Computer
Self Motivator
Quick Learner
Team Player
Potentials to grow

Rating of 1-5 may given to above attributes. Based on nature of work /Industry requirement additional attributes can be added /removed.

It is very common to ask the family details of the candidate at the interview stage to know about his/her family commitments.
