Cancel/deregister Allotted Provident Fund Registration?

Our establishment is a partnership firm. The said firm closed all its activities since 2013. We have been allotted provident fund registration which we want to cancel/de-register. Kindly give us the procedure and the formalities

cancel your trade licence and close bank account, submit both documents to EPFO with request to close the establishment closure proceeding
upto the month of this formality the establishment has to pay a/c no 2 and 22 ....amount 75 and 25 ( total 100/-) per month
