There are two forms viz;
Form 10D is related to the non- executed nominee by an employee. who should be certified by the family members issued by employer or the legal certificate from the court. It is available to claim pension. (Form-10-D : To claim pension. (In duplicate : If within state, In triplicate; If outside state )).
Form 10 has to be applied by the employee for claiming pension by showing the details of leaving service and details of contribution for the years worked in Form 3A in case if the employer fails to send the details.(Form-10-C : To claim withdrawal benefit/scheme certificate under Employees' Pension Scheme).
