As you specified as you need the registration for partnership firm below are the required documents along with the application forms
ESI
Application should be filled online and below documents should be submitted
1) Copy Partnership Registration Certificate
2) Copies of address & ID proof of all Partners
3) PAN Card Copies of partners
4) Firms Address proof & PAN Card
5) Firms Electricity Bill
6) Attendance List for last 2 months
7) Salary Statement for last 2 months
8) Any sales invoice or work order
the same documents for PF too
