yhaa, accident prevention is a very sencitive topic, in safety everything carries equval importance. bt this is the life saving one, and it shows its effect immediately.

so i wish to study and learn more in this area, I belive the term " ALL ACCIDENTS ARE PREVENTABLE "

I HAVE SEVERAL QUESTIONARIES.

1.WHAT ARE THE MAJOUR CAUSES FOR ACCIDENT?

it varries according to process and work activity.

in general THE MAJOUR CAUSE IS UNSAFE ACT/ other fectors 80/20.

NOW 2. WHAT ARE THE MAJOR FACTORS LEADS TO UNSAFE ACTS?

it may gender , age , work load factors, etc.

THEN 3. WHAT ARE THE MITIGATION MEASURES FOR THESE UNSAFE ACTS?

training and awareness THEN WHY SKILL BASED ERROR ie SLIP OF ACTION.

etc.

BY BEHAVIOUR BASED SAFETY CONSEPT WE CAN ADDRESS MOST OF THIS ISSUSE.

THIS IS HUMAN FAILURES,

4. NOW WHAT ARE THE CAUSES FOR MEACHINE OR PROCESS OR ANY ACTIVITY FAILURE ?

like mintaneance, poor guard or poor design, or failure of devices etc.

ok we can identify all potential hazards easily by audit findings or by experiance...



NOW MY REQUIREMENT IS THER ANY OTHER NEW PROCEDURE FOR IDENTIFYING ALL THE POTENTIAL HAZARDS IN THE INDUSTRIAL PREMISES?


like creating safety culture for reporting every near miss and incident with out fault finding . pre operation and post operation user checklists before starting the work and ending the shift.

AS U SAID ITS NOT EASY TO GET ALL ACCIDENT RECORDS.

so i wish to know THE EFFECTIVE METHODOLIGY TO IDENTIFY ALL POTENTIAL HAZARDS THEN IT MAY LIT BIT EASY TO INSTALL CONTROL MEASURES and that too is cost effective...

Before trying to answer your query I would like to mention few things:

1. If you expecting reply from me only towards your query just send me a mail instead of starting up a thread addressing me only... Our other forum member will feel odd on such kind of communication and they will not come up with their views... So let it be an open thread with a hope that some of our forum member will help you in replying to the thread...

2. If so particular after start a thread you can send a private message asking for participation in particular thread

3. In this thread you have come up with many issues... For a member to participate and give a satisfactory answer to you, he/she have to spend a good amount of time in it... So try to be very short in your posting... May be two or three queries/concerns at a time... This will increase the participation level, I am sure...

Now let me share my views on what you asked:

We believe that there are following three causes behind every incident, even if it’s a Near Miss:

1. Physical Cause

2. Human Cause

3. System Cause

It’s not always the employees doing mistake... Why your systems allowing them to do mistake?

NOW 2. WHAT ARE THE MAJOR FACTORS LEADS TO UNSAFE ACTS?

Many factors starting from the mental stress of an employee... You may refer the attachment which I downloaded from net only...

THEN 3. WHAT ARE THE MITIGATION MEASURES FOR THESE UNSAFE ACTS?

As you rightly mention about training, behavioural based safety, etc is very good tool...

4. NOW WHAT ARE THE CAUSES FOR MEACHINE OR PROCESS OR ANY ACTIVITY FAILURE ?

It’s because of the lack of Operating Discipline... It’s the key to safety for running plants... Covering broad concepts starting from just housekeeping to preventive maintenance...

Dear All,
For the last point of identifying ALL the hazards, I have some thing to add.
1. Identification of hazards is an ongoing process. It can be reactive as well as proactive.
2. Reactive Type: As and when there is any near-miss, incident etc. the hazard for that activity is to be identified.
3. Proactive Type : In addition to HIRA, JSA, you can also try HAZOP, FTA, ETA (I remember to have read on Internet “The Columbia satellite accident in which Kalpana Chawala died along with 6 other astronauts on Feb 1,2003; the FTA was not complete and had an open end.)
4. In design stage latest tools are FMEA, FEA.
5. Implementation of ISO 28001 which is on security managements to counter terrorist threats. I am sure we would not have imagined about this hazard but it has become vital after twin towers attach.

i am still not very convinced of the tools like ETA, FTA, RA, etc being used indiscriminately without understanding the requirement.Have been more with the conventional school of safety management having led programs for completing millions of accident free man hours in large companies like Tata Steel, Torrent Power, and Reliance in India. I recommend simple programs like JSA interlaced with a very accurately done Hazards Catching Program in a company with a mission of achieving in steps firstly one million and then progressively to ten million accident free man hours. At each step we must do a full review of our existing practices and near misses which might have jeopardized the achieving of the targets. Never budget accidents!!!! It should always be Zero.
