I'm doing my PhD in Employee Assistance Program. Now I am in the stage of preparing Research Data through Questionnaires (completed Course Work, etc.,). Can anybody help me with different models of Questionnaires, which will be really useful for me.

Your post is a bit confusing to me, as you say that you are "in the stage of preparing Research Data ...." What do you mean by completed course work? Where are you pursuing your PhD, and in what mode?
What are your aims, objectives and hypotheses? Before arriving at these have you not done the literature survey to see what others have done, how and why that way? If you have, did you not come across any questionnaires used by others?
