The important thing about any vehicle is whether its 1st line servicing is done or not. Driver is supposed to do the 1st line servicing. Secondly, what about vehicle's monthly checks and other periodicals checks? What is the servicing schedule?
Another thing is about mileage. You need to record the mileage also.
Lastly, when the vehicle was purchased and what are the expenses on repairs and maintenance? Is the retention of the vehicle costly? Vehicle is a movable capital equipment and we have to depreciate its value every year. Now we must ask a question to ourself whether we are overspending on depreciated asset.
When you say "audit", financial audit surpass all other points.

I had gone through your TRANSPORT AUDIT FORM which sounds to me a "CHECK LIST" but not able to understand its MAJOR PURPOSE OF AUDIT, where you had included the factors associated with DRIVER rather focusing on COMMERCIALS ASSOCIATED with VEHICLE, where Mr. Dinesh had provided its inputs.
I second to Mr. Dinesh, as the TRANSPORT AUDIT FORM should be correlated with FLEET MANAGEMENT that narrates VEHICLE HISTORY right from PURCHASE DATE to TILL DATE VEHICLE CONDITION including ALL SERVICES DONE by/at INDIVIDUAL/PERIODICAL LEVELS, COST INCURRED TO MAINTAIN VEHICLE(including replacement of spares)
Kindly correct me if i am wrong. I suggest all those to modify TRANSPORT AUDIT FORM based on your requirements.
