REGISTRATION WITH THE ESI AUTHORITIES FOR ALLOTMENT OF CODE TO AN ORGANISATION TO WHICH ESI ACT HAS BECOME APPLICABLE IS A SIMPLE PROCESS BY USING FORM 01 WHICH IS ALSO CALLED EMPLOYER'S REGISTRATION FORM. Hi,
You may submit form 01 of ESI for registration with details of the employees (already mentioned on the form) at ESI Regional Commissioner's office.
In case of ESI, you may register online also by visiting the URL: www.esic.nic.in
You may submit "Proforma for Registration" of EPF along with Form for allotment of Business Number with relevant details as mentioned in the form at PF Commissioner's office.
Further you may read another posting with name of PF ESI end to end process where almost 100s of people have shown their experience and solved various professional queries related to this topic.

First of all, you need to have the requisite number of employees in your organisation.

Apply with proof of your office (date of establishment, Sales Tax/ Service Tax, bank statement, balance sheet etc.) These will be verified by the inspector who will inspect the establishment and approve your application. (inspection means you know ........)

Many fly by night companies, due to mandatory requirements for bagging some contracts, get the number and do not pay afterwards and are not tracable.

Please mention all the details (including number of employees for the past) the same in both applications.

Once a number is alloted, start paying in SBI every month before 15th of every month. Challans will be available after 15 days. Collect all these challans and submit a copy along with Annual return for EPF and half yearly returns for ESIC.

ESIC may be alloted without much difficulties. They may not visit your establishment. It wouldbe advisable not to contact any agents as they may fllece you.

Most ESIC and EPF employees also work as consultants. So who ever you approach will tell you that they will do all work.

EPF returns is a simple procedure. But ESIC involves much complications also. So if you intend to have a consultant, have them from ESIC background.
