The format for the staff movement in/out register is as per the format below in columns :
Sl No., Date, Name, Designation/Dept, Purpose, Authority if required, Time Out, Time In, Signature of Employee, Signature of Security/Others.
Draw these columns in a register or get it printed as required.
If attendance sheet is maintained by the Security people then you can make a outpass and then the same can be entered in a separate sheet along with the day attendance sheet and on the next day will be handed over to hr/admin department.
It depends upon the working structure of the co.
Anyhow you can clarify further if required.

Why you have to increase the paperwork. There is many systems available with the punching system. If an employee has to move out of the company on official work or on personal work he can punch his card along with the OD card. You can keep a puncing device at your security/time office for monitoring the staff movement. A daily/weekly/monthly report also can be taken out. Even the data can be stored for years.
