Friends,
Why-Why analysis is a tool to identify root causes of a problem so that countermeasures can be applied to prevent reoccurences. Can someone throw more light on this topic? If you have any specific examples, formats, tools that can help to broaden the understanding on this concept please do share.....

Dear Shijit,
Please search the web and you will find many examples. If you do not succeeed, I will help you.
To start with, you could ask the question "Why am I posing this question at CiteHr instead of searching the web using Google?" Then when you give a reason ask "Why?" again. Keep repeating that and you will eventually get to the root cause.
