Cany any one help me in formulating a questionnaire for outsourced service evaluation? Anyprocedure for the evaluation? Is there any tips for this in ISO certified institutions? Ajay

Hi Ajay,
Attached is the format for the evaluation whci can be modified as per the requirement and also in the header / footer , whenever u make anychanges in the form revision number needs to be changed thats the ISO thing.
More help required do ask for.
