It is Form 15 (Leave with Wages) instead of form 14. Please find attached the form 15 (Register of leave with wages).

Please find attached herewith filled up Form no 15 Leave with Wages Register prescribed under the Factories Act, 1948. The manual calculation in this register is slightly difference then of the computer. The formula for calculation of PL is one day leave upon every 20 working days. I am explaining the method in which you can calculate earn leaves and maintain these in leave with wages register
suppose if an employee has worked from 01/01/2012 to 30/06/2012 as under
Month Days worked Balance leave as on close of the previous month Current month credit leave availed in month balance leave
Jan 25 0 1.05 0 1.05
Feb 24 1.05 1.04 0 2.09
Mar 25 2.09 1.05 0 3.14
Apr 26 3.14 1.06 0 5.00
May 25 5.00 1.05 0 6.05
Jun 24 6.05 1.04 2 5.09
This is the manual method you can calculate leave for an employee.
