Could anyone please help me out in how to prepare a performance mangement questionnaire? What are the important topics to be addressed to employees to find out whether they are satisfied with the current performance management process

basically for questionnaire you have to see first which were prime objective of setting PMS in your company.......than you took out PMS steps and sub steps.......... than find out hw it is related to employee satisfaction and prepare questionnaire putting your self as an employee .........
