The mode of submission depends as per your states' guideline, but of course it is mandatory to submit Form C if your company/establishment is covered under payment of Bonus Act.

Dear Shailesh,
W are to keep Payment of Bonus Register in Form C which is not to be sent to any of the Authority. We are required to submit annual return under this Act in Form D to the Inspector of the area.

You are required to maintain not only Form C but Form A & B also under payment of bonus act. The GLO (officials from Labour office) always asks for these forms when they come for inspections. Form D is now part of Consolidated Annual Return to be filed by employer and is to be submitted (which is now available online).
