Dear All, Please share the procedure to get ESIC Card. Waiting for yoour early reply.

First of all clarify, are you asking from employer point of view or emp[loyee point of view.

Some basic requirement are as follows

The company must have more than 20 employees and registration with ESIC thereafter.

The employees gross salary should not ecxeed 15000 permonth

After joining a firm, the employees need to fill adeclaration form and give family photograph and submit to the employer.

The forms are then signed and stamped by company and sent to ESI office through concerned person or consultant.

The temporary card is given within a weeks time by esic office to concerned person or consultant.

The permanent card is sent to your address mentioned in the declaration form along with the ESI stamp on the photograph provided by you.

This permanent card is one time and can be used with every / any emploer throu transfer of employer.

The permanent card is invalid without form 37 which is given by employer evry six months i.e octoer and april, this card states that you are working with particular employer.

Thanks for your reply.
I am working in a company which Registered in Delhi with ESIC. Some time before i asked to our HR deptt for ESI Card. I have filled the form as provided by them and sent them with my post card size photograph.
They got that form attestted from ESIC deptt delhi. And asked me to go Local office for ESI Card. I am working in Karnal Branch. And Local office for Karnal and Panipat (Haryana) is in Panipat. Yesterday I went there in the local office. I had asked to the concerned person for making ESI Card. But he requires Insurance No which is in 10 digit. When I asked our HR deptt to share the same. They said that insurance no is mentioned on the form which is 2013560301.
But the concerned peron is not accepting this.
Kindly help for the same.

hi,
After going through your letter i come to know that your office is having register office at Delhi the company must be having the ESI code at his Delhi office addres. you are working at Haryana so your company must apply for the sub code at panipat then you can register your self at panipat.

Dear Mr Rajekashyap
Ms Yaasmin has answered your queries correctly.
It will be the employer's duty to get the ESI card to its employees and employee need not go to ESI office
However, you may support the concerned case worker and take self-initiative to talk/go to ESI local office/branch office
If ESI number is not available and in case of industrial accidents, the employer will be held responsible !!

Dear member, it is the Insured Person himself/herself to apply for the Duplicate Identity Card to the branch office to which he/she is attached. He/she may be advised to contact the appropriate Branch Office of ESIC and fill up some form for issue of duplicate identity card (DIC). I think, there is some nominal fee ( earlier it was Rs.1/- only), which is to be paid by the said employee to Branch Office of ESIC. I think, it is easy process.

Sir, I think, the system of issue of TIC or Permanent Identity Card for ESIC covered employees cannot be stopped because such documents are required to be shown to authorities in ESI Dispensary/ Branch Office/ ESIC Hospitals etc. etc. Hence I will suggest you to contact your concerned official in factory dealing with ESIC or Branch Office Manager of ESIC to which your unit is attached and demand for such document as above.
