There wont be no special format for this
you cannot get this format any where or even in statutory legal book shops
it is the inspection observation to be written by the inspector
this is called for Form 103 under ESI act
just buy a ordinary 80 or 100 pages small size note book on top of the first page write ESI inspection hand book (Form 103)
this is just a register , where in the esi inspector will right is remarks

1. Sir, ESI Inspection Book, ( though not having prescribed proforma) is generally available on leading stationery book stores (which supply such registers to be maintained under various labour laws including Factory Act). You can confirm the same from such stationery stores in your city.
2. Otherwise you can adopt the method as suggested by Sh. R.Sudhar ji in his remarks as above.
