Dear Sachin,

Your post is a little confusing. Why do you want the format of the "Asset Declaration Form"? Are you from a private organisation or a government organisation? What is your designation?

Assets are required to be declared every year only in government organisations. However, in private companies, I never heard anything of that kind. When a company "allots" certain assets to the company, it obtains from the employee, "Asset Allocation Form" and not another way around.

Does your company have a policy on the allotment of the company's assets? If not, you need to design first that. You also need to maintain a proper Asset Register. Unless you do all this your problems may not be solved as such.

Its little bit confusing...are you looking for investment declaration form or asset declaration form?

Dear Sachin,
As rightly pointed out by Mr. Dinesh, the asset declaration form is required to be given in Govt. organizations only. Without much technicalities, I would like to share that : There are two types of declarations to be made - one is for moveable and the other for immovable assets. The property or assets acquired during the calender year in question. Past assets already declared need not be repeated.
For movable property : a)Type of asset acquired(FD/Vehicle/Shares or alike)/Date of acquisition/Value/Beneficiary name(Self, spouse or children).
b) Type of asset sold/Date of sell/ Value/ Beneficiary name.
For Immovable Property : Type of asset/Details of Asset(Address, Plot No., etc.)/Date of acquisition/Value/Beneficiary Name
Similarly for asset(s) disposed off during the calender year.

Dear friend,
As others said, the format could be anything according to your requirement. Normally this is in practice in govt./quasi govt./PSUs establishments. Nowadays some pvt.MNCs also started obtaining such confidential information. You can use a format which is being used by us with or without modifications as may be required. Of course there has been controversies emanating from such declarations as the legality and how to deal with omissions and commissions. In case where these declarations are being followed any updation of these information should be intimated to the respective offices within some prescribed period (say within 30 days of relevant happenings).
