However in general you can have a systematic approach by taking the service of a qualified safety personnel.
In a broad sense look for
UNSAFE ACTS &
UNSAFE PRACTICES.
Once the above two are known you can consider implementing the corrective action/ mitigation plan after taking the help of functional team.
The monitoring as above is a continuous process.
