Do we all still follow the age old excel or word document evaluation sheets during the interviews or is there an better way to do this? Does anyone know of a software tool that makes it a simpler and cleaner process?

Dear Mr.Suddsvr,
In recent times there have been many technological softwares that support human life at ease. Besides, Jeevarathnam ji, said rightly about "investments" -- If the employer had to pay it all then I think they dont need Humans to work on. Machines and Automaton will reign the source.
Human Resource is at beauty and center-stage only when employers see them right. Hence, to maintain an employee record or file system one needs to use nothing else but [A WORD DOCUMENT].

Dear Suddsvr,
Are you asking any question to know for your market research? Is the underlying idea behind asking question is to promote your own product? I ask this question because you have not shared any of your personal details like the type of company in which you work, your designation etc. You have concealed even your name also.
Secondly, you have not written limitations that you face by using forms made of MS Word or MS Excel.
Good Documents is best and free of cost also. There is a provision to create form on Google Docs. Create customised form and you can retain it for ages. Since these are online forms, sharing the forms is much easier.
Lastly, my dear friend let me tell you that job interview is filtration process and this filtration happens best when interviewer asks quality questions. Retention of Interview Evaluation Sheet is only a clerical activity in a larger part of recruitment process.
Ok...

Dear Dinesh,
Its funny how instead of seeing a question in good light, we start popping up with additional questions, doubting why a person asked a question, was it with hidden motives to promote a product, or spy on our information. Skepticism runs deep in our society. Take it light Dinesh. If I had to promote a product, I'll write about it upfront.
Since you need detailed information about me to answer a simple question, here is some food for thought. I am an Indian who speaks hindi and eats vegetarian food and runs a travel company. Should you need further information that may help you share a productive answer, I'll be happy to share.
Regards,
Sudds

Evaluation Sheets during interview:
for what tests? using an office copier worth 1.5 lakhs which is already invested by our office for printing scanning etc, and using a free source OMR Software we conduct recruitment tests for our exams and results for 1000 candidates are evaluated, corrected and published in 20 mins.
If you are having a good quality fast ADF scanner you can download a free OMR answer sheet sofware and use it to conduct objective type recruitment tests only.
Hope this is what you are looking for.
