Part A, the member can fill with percent mentioned clearly
Part B 1 is usually for spouse and children, however unmarried can fill the parent's details
Part B 2 if there is no specified family member (i.e B-1 is Blank) , viz parent, wife, or children, then the member can nominate, brother/ sister or any other blood relative, with their details
