Dear Ashish,
For the reemployment of the terminated employee, there need not be a different format of the appointment letter. You can keep the same format of the appointment letter.
However, you have not mentioned the reason for the reemployment. Why the need was felt to reappoint a terminated employee? Is it out of the generosity of the management or was it a wrongful termination? Inclusion of the additional clause suggested provided we know the exact reason for the reemployment.
Thanks,

Reemployment means it is new employment for all intents and purposes. So a fresh appointment order as any other case is advisable. If the old employment is linked in any manner, later there could be complex issues of seniority and related matters.

Dear colleague,
If you are rehiring terminated employee ignoring his past tainted history, it will be tanatamount to fresh appointment like any other new appointees. So appointment letter will not be any different from the one being currently issued to them.
You need no different format of appointment letter to be issued to him.

Dear Ashish,
As advised by Seniors, Appointment Letter Format is the same.
However, before issuing him the same ensure that you take written application from him for an employment alongwith his CV. Also give him new employee ID/Code Number.
Dear Mr. Vinayak Nagarkar,
Heartiest Congratulations for scoring "Triple Chauka"!!!!
All the very Best.
