Not form 4 (As told by Mr V. K. SHARMA), but form 2 three copies (if not changed) with origional payment receipt of bank and origional Licence of last year (if establishment/factory already registered) to be sent to the Deputy Director of Factories of your area before December 1 each year

I am working In MP and as per our State rules it is form 4. As per your stste rules, it can be Form 2 and it can be different in that stste , where Mr. Pandit is working. It would be better, if we may know the correct Name of his state.

The contents of the renewal form are almost same but its numbering is different which may be form 2,3 or 4 depending upon the State Factories Rules. Moreover, now few states are doing the renewal online.
