Dear all,
Near miss reporting plays a very important role to develope safety culture.
But still people dont report them.
So how can we encourage the people or motivate them to report more & more Near Miss.
Looking forward for your valuable suggestions.

My views on your query:

First of all you have to find out the answer why the people not reporting the Incidents. Whether Near Miss or Other Incidents.

Most the cases the answer will be the following:

1. Fear of Punitive Action

2. Reporting process may complicated.

3. No benefit will get even if I report.

As a soluting to these problems:

1. We have to remove the fear factor from the peoples mind. For that a circular from the top management clearly mentioning there will be no punitive action against reporting incidents. Also include that if any incident which took place in your area of supervision and the same not reported then management will take desplinary action against that. This will surely help. Further detailed training highlighting the need of reporting all incidents should give to all concerned. A circular only will not help in change of situation. Training is very much required.

2. Make the reporting format very simple. If required print both in Hindi and English . So that even a supervisor can fill it.

3. May introduce a reward system for reporting Incidents. 100 rs against reporting near miss and all.

thanks for prompt reply..
yes i agree fear factor may be one of the reasons though we encourage people that no action will be taken against them.
still people hesitate to report.
here in our plant reporting form is there whch is avaliable online also.but ye not in hindi.this could be a good suggestion so that even supervisiors can report.
We also have award of Rs. 100 for reporting of near miss.
In training i do tell them importance & advantages of reporting near miss.
lets see if we could get some more suggestions from other experts also.

near miss incidents do have the issue of having punitive reverbations. However the organisations can develop a intra portal wherein the near miss incidents and how to avoid the same in brief can be updated. The Senior Management team has to take a conscious stand of encouraging the same rather than looking for punitive opportunity

Untill and unless the fear factor not gone away from the mind of people, you can't improve reporting system... If you punish anyone against reporting an Incident, even if the fault is with that person, then next time onwards who will be interested to put themselves in troubles by reporting something... For improving this, a real committment needs from the top management... Whatever incident reporting, must be investigate in detail to find out the contributory reasons, ie. all Physical, Human as well as System Factors. And the management must shown interest to implement the corrective and preventive actions arising out of this investigation to ensure that such kind of incidents not repeat in future...

If any managemet is giving any punitive action to a person against reporting incidents happened, as per my understanding its wrong... We are behind the Human factor which causes the particular incident... Just closing eyes against the contributory Physical as well as System factors... In this condition chances of repeating such kind incidents are high... Hence we are just killing all the real intension of even improving reporting system...

Any Rule... Untill and Unless is Written... Is not a rule... Will not affect human minds... Which is full of his own theory and mentality... Not so easy to change...

I agree with Dilip. We have online system of Near Miss reporting and single page format available at various location to be filled by contractor staff. Safety Boxes are placed throughout at various location. Person may write his name which is optional. Best near miss is given award (monthly) and all reported nearmiss is given beautiful pen.

All nearmiss reported can be viewed online and CORRECTIVE ACTIONS and present status is also mentioned. Our management fully supports for taking immediate actions and then corrective actions (audit similar conditions throughout the plant and various locations)

The best of best Near Miss reported is awarded during Safety Day.

I would suggest that apart from what Dilip & other team members suggested show the employees that Corrective actions are taken and you (Safety) are really serious on taking the actions. You may mail to all users in your unit regarding nearmiss reported - forward path - corrective action suggested - target date and then completion mail. Also request all users to report similar nearmiss chances in their respective units for rectification to be done.

You may also introduce NEAR MISS theme for the month and give rolling trophy to the department reporting maximum nearmiss.

Any Photographs, Cartoons, to display which itself reflects to workers the meaning of Near Miss... My main aim is to make workers understand what's mean by Near Miss Throguh Visual sign boards...

Hi Hansa,

Not only you facing an above mentioned subject problem in your work area. Most of them having a same problem in Industries specially in construction industry.

Let me share my point of view on this topic;

Take time with your employees and supervisors to explain why near miss reporting is beneficial.

Provide a simple, clear process for reporting and investigating near misses.

Supervisors should play an main role in near miss reporting and provide clear briefing material to supervisors who are directly engaged with employees most of the time.

Provide reassurance that reporting a near miss is something to be celebrated - This is very Important which will make difference in our work area

Follow up by responding visibly and positively to near miss reports. It must be available and easily accessible to all.

All Near miss should be investigated and corrective action taken all the corrective action that you taken must be available in work area to all employees

Should be follow up with the individual or with his supervisor who was reported the near miss.

Most important is you need to find the way how to motivate your employees in your work place to report near miss. - I'm in construction its totally different environment than industries.

How to motivate the employees.

Supervisors must be talk to all the employees and explain the benefits of near miss reporting.

Pass the email to all who reported the near miss with his photo so he will become famous in our company - It wont work in most of places.

As you said you conducting an training to your employees make sure your training reached them to all LEVEL of employees.

Provide a T-Shirts (with some nice LOGO) who reported large number of near misses every week or every month and send a congrats circular email to all the employees make sure your TOP MANAGEMENT should reply on the email at all times to appreciate the individual and the department.

Conduct a Near miss contest in every department.
