1. Sir, it will be more better if you ask your employer/HR to supply the copy of monthly return and challan being paid by them to ESIC online. From monthly return of any month, you can see your details. The same can also be viewed online and I think, you can ascertain the procedure by contacting your Branch Office of ESIC.
2. If the employer fails to provide you above information, you can ask the same from appropriate Branch Office of ESIC to which you are attached. You can also obtain such informations under RTI from Branch Office Manager, ESIC of your area, who I think is the CPIO under said Act.
