I am working for a manufacturing company.
I have been asked to design an induction feedback form for new officers joining our plant.
In the induction feedback form, I have been asked to cover areas from initial reaction while joining, overall admin support recieved to understanding of job roles etc.
I have been asked to design a feedback form which will be administered to the new joinees 10 days, 1 month & 3 months after their joining.
Could any one please help me with questions that should be asked to the new joinees?

When and how was the co. started?
What are the career growths for the new joinees in the co.?
How the salary structure would be showing an hike in the industry?
How the performance is calculated?
Any bonus given based on the performance?
