How can employee update correct date of exit in EPF records?

The date of exit of one our company’s employees is not correct. Due to this, he is unable to transfer his PF. How can he update his correct date of exit in the EPF records?

The employee needs to fill up the "joint declaration" form and the employer needs to duly sign it and submit at EPFO for amendments to date of exit.
