Dear Vipla,
Registers are required to be kept according to the State Factory Rules & therefore go through the relevant rules of the State where the unit is functioning for which you are to look after the work of Factory Act compliances. You can also take the help from your seniors.

Dear Sangamesh,
Format of registers are given in the Rules of different Acts as applicable to your unit & if you are covered under all the four Acts as mentioned then go through central Rules on ESI & PF Act & your State applicable Rules on Contract Labour( R &A) Act and Factories Act for the formats for maintaining the registers.

I am not sure if Factory act and shop and Est. Acts are same but Shop & Establishment act is as per state.
You easily get act information in any stationery shop, If anybody need details about Bombay Shop & Establishment act... please find attached file.
