Khothilda, I think you didn’t seen the website given by us properly. There is a sample questionnaire on Leadership styles. Convert that questionnaire as per your requirements.

ok..thanks..tis is my first time doing thesis so quite difficult for me as my lecturer wants me to find an existing questionnaire used by others and it must be valid and reliable.tats y i cants simply choose any questionnaire from the net..will try again.thanks.

I never asked you to find any questionnaire. When people publish articles in scholarly journals, they usually state whether the questionnaire has been tested for validity and reliability. However, a questionnaire that has been validated in USA or UK may not be valid for Indian conditions and you will have to subject the same to tests.
