The only hassle with your format is that the observation is not objective enough. We could consider quantifying each attribute by awarding marks. However, the maximum marks for each attribute have to be determined, which would then total up to an overall score (possibly out of 100). This is just an idea. You could also compare teachers' performances and grade them within the staff in order of merit.

However, when the same observation report is used for evaluating a teacher for promotion or a salary increase, it needs to be quantitative.
